You should recieve a hard copy consultation form to send back but here are other ways to respond to the threatened A&E and Maternity services from the PRH campaign team. As the email circular says, don't rely on others doing it for you, they may be relying on you!
The Primary Care Trust consultation closes at 9am next Wednesday, 14th November. For the campaign to succeed, it is vital that local people respond personally to the consultation. Please take 5 minutes to write your response to the PCT. Here are 5 ways you can respond:

A leaflet from the campaign is currently being delivered to tens of thousands of households in Mid Sussex. The rear page contains a response form for residents to complete and return to the PCT free of charge.
